Why Do I Feel Anxious for No Reason? The 9 Types of Anxiety from Panic Proof, with Dr. Nicole Cain (Part 1)

This is Part 1 of a two part conversation.


You feel anxious. Nothing has happened. Nothing is wrong on paper.

That gap is where most people give up on the question. If there is no reason out there, the reason must be you.


I want to offer a different place to look.


In surgical residency, I covered the emergency room. Someone would come in and say they were anxious. My job in that moment was to work out why. The answer changed everything.


It could be a person having a hard week. Go home, take a walk, have a bath. It could also be a heart attack. Same word from the patient. Two entirely different bodies in front of me.


I still run that process on myself. Anxiety starts humming in the background of an ordinary day. I go through a checklist. Was it something I ate. Did I overextend myself this week. Is something surfacing that I have been avoiding.


Then I read Dr. Nicole Cain's book, Panic Proof. She had built the same checklist, and made it far more complete. Nine types. Each one named for the body system driving it.

This conversation is the first half of that map.


Anxiety that seems to come from nowhere has a starting place in your body. Where you feel it first points to which system is speaking. Cardiac, gut, and thought anxiety each carry different causes. Each asks something different of you.




































WHY DO I FEEL ANXIOUS FOR NO REASON?


Because the reason is not in your circumstances. It is in a body system that adapted to something. That adaptation is still running.


Your body responded to something real, once. That response was intelligent when it formed. It held past the moment that created it.


Now it produces a signal that no longer matches your life. From the inside, that reads as anxiety with no cause.


There is a cause. It is somewhere other than where you have been looking.


WHERE IS MY ANXIETY COMING FROM?


Start with location, and start with the first moment.


Dr. Cain calls that first signal your canary. Anxiety that begins in your chest points at one system. Anxiety that begins in your gut points at another. Anxiety that arrives as looping thoughts points at a third.


By the time it reaches full intensity, everything from head to toe feels off. That is why the first moment carries the information. The early signal is still specific.


This is also why one answer for everyone tends to disappoint. Dr. Cain describes anxiety being handled as a single diagnosis, with general treatments giving general answers. Her question instead was what the body was actually saying.


WHAT ARE THE NINE TYPES OF ANXIETY IN PANIC PROOF?


They are nine body systems. Each one can produce anxiety. Each one needs something different.


Dr. Cain is clear that these are not diagnostic categories. They are a working map for finding where to focus.

HOW DO I KNOW THE SOURCE OF MY ANXIETY?


Notice where it begins, before it spreads, and ask what it is an adaptation to.

Those are two questions and they work in that order.


The first one sorts. Chest, gut, or thoughts. It also does something beyond sorting. It puts you in the position of noticing rather than being carried.


The second question is Dr. Cain's, and it is the one that changes the work. Not what is wrong with me. What is this an adaptation to.


We get to ask better questions of our own bodies. That is where becoming a better partner to yourself starts.


WHAT IS THE DIFFERENCE BETWEEN FEAR, ANXIETY, AND PANIC?


They are three different states, and they sit on one line.


Fear happens in real danger. The body mounts a full response before thinking catches up. Hormones shift. The immune system responds. Fear is doing its job.

Anxiety is that same response outside of real danger. Traffic. A deadline. A presentation that has not happened yet.


Panic sits at the far end of the same line. Dr. Cain describes a green zone where you feel grounded. Then a yellow zone, where something registers as off and the signals get louder. Then a red zone, where everything in you is fighting for safety.


The value of the map is early detection. As she puts it, catching the whispers often stops them from becoming shouts.


AM I IN HIGH ENERGY ANXIETY, OR HAVE I CROSSED INTO SHUTDOWN?


Both get called anxiety, and they need opposite things.

This is the distinction I care about most. It is also the one most often missed.


There is a critical line of overwhelm. On one side, your body runs on activation. You feel the pull to move, to do, to keep going. On the other side, your body has crossed over. Anxiety is still there. So is hopelessness, fatigue, and the pull to lie down.


Dr. Cain adds three responses beyond fight, flight, and freeze. Flop, fawn, and fracture. In a flop, blood pressure can drop. Dizziness and exhaustion follow.


She hears one version constantly from women in perimenopause. They have been stressed their whole lives, and now they cannot do it anymore. The pull is to curl up on the couch.


Asking a body in flop to push harder does damage. That body reached the end of its capacity and applied the brake itself. Your nervous system said it would stop you. You were not going to stop yourself.


WHY DO I FEEL ANXIETY IN MY CHEST?


Because the chest holds the physical signal, and what you have not felt.


Rule out the heart first. Dr. Cain is direct about that, and so am I. Cardiac workup, exam, imaging.


After that, the chest is worth listening to. She sees this pattern in athletes after retirement. The body had adapted to one baseline of breathing and cardiac function. When the baseline changed, the system had to recalibrate.


I see the same shape in executives who retire. They arrive convinced they are having a heart attack, and they use the word panic. What they are meeting is stillness, and everything the busyness had been holding down. It surfaces, and it surfaces in the chest.


CAN ANXIETY COME FROM MY GUT?


Yes, and often on a delay that makes it hard to catch.


The gut and brain talk in both directions. Dr. Cain notes that roughly 80 percent of that traffic runs upward, from the gut.


My own turning point was a food and mood journal. It surfaced a pattern I had missed for years, because the reaction was delayed. My mood would drop a day or two after eating gluten. Never the same day.


That delay is the whole point. Slow reactions do not announce themselves. A written record is what makes them visible.


Dr. Cain calls this the nuance of a cascade. The body is not only producing immediate reactions. It produces sequences that unfold over days.


WHY WON'T MY THOUGHTS STOP LOOPING?


Because stress routes the brain around the part that could work with the thought.

Looping thoughts. Racing speech. A mind that goes blank when someone asks a direct question. Word hunting.


The usual advice is to talk yourself out of it. Dr. Cain asks what the thought is an adaptation to. And what holds it in place.


When stress is high, the network that handles careful thinking gets bypassed. The stress pathways take over. That is why settling the body has to come before working with the thought.


This is where I bring in parts work, and I consider it essential. Without it, we are at the mercy of our thoughts.


Naming a thought as belonging to a younger part of you changes its authority. The inner critic stops being the truth about who you are.


My son Miguel is where I watched this land. He could see that losing control was not his present self. It was a younger part who had lost its sense of safety. Once he could see it, he had a choice. He did not have that a moment before.


HOW MUCH OF THIS IS STORED TRAUMA?


Anxiety is the clearest expression of Dysregulation, one of the five patterns stored trauma produces.


Dysregulation looks like a body swinging between too much and too little. It never finds the middle.


Disconnection often travels with it. Not reading your gut's signals is a form of distance from your own body. So is feeling carried by thoughts that seem to arrive from nowhere.

The mechanism underneath both is the same. Your body adapted. The adaptation held. The system keeps repeating what it adapted to, and that repetition has a cost.


Sequence matters here. Safety comes before support, and support comes before expansion. A body without felt safety cannot do the deeper work. Willingness does not substitute for it.
